Forensic Case Manager — Jail Diversion Program — Collaborative Justice Services Provide effective discharge planning, linkage and referral to community resources for mentally ill offenders who re-enter the community following release from the county correctional system and jail diversion from local municipalities. Successful participants have the opportunity not only to turn their lives around, but to have a reduction or dismissal of their criminal charges.
COIN participants receive residential substance abuse services from AVRC staff while receiving case management and mental health treatment from Project Participants are identified in the emergency department setting, and are linked and transported directly into residential treatment at AVRC.
Participants stay for three months, and are provided with mental health services by Project if they are co-occurring. The program provides both interim and permanent supportive housing for some of the most underserved, or highest utilizers in the County system. Links to Health also known as Behavioral Health Home JFS Links to Health is a Behavioral Health Home model that offers healthcare connections, care coordination, linkages, coaching, mental health and medical services as well as wellness workshops.
The program benefits participants through a continued level of care that help individuals stay healthy and out of the hospital and emergency room. Links to Health also prioritizes the facilitation of communication among behavioral health and medical care providers in both acute and on an ongoing basis.
Jail Reentry Program Through a contact with Atlantic County, the program provides wrap-around discharge planning and reentry support for all county sentenced inmates includes all of those anticipated to be county sentenced. The Jail Reentry staff conduct bio-psychosocial assessments that inform a reentry plan for clients.
